However, if none of these can be contacted I hereby authorise the service to transport my child to the doctor's surgery or to the appropriate hospital A/E department by ambulance or as is necessary and to secure the necessary medicaltreatmentformychild.
I give my permission for my child to be given Calpol or Nurofen in case of high fever/ severe pain. Sherpa Kids will always talk on the phone with the parents prior to administering the medicine and will only do so according to the dosage and instructions written on the package.
I hereby give permission for my child to watch G & PG rated movies and games.
School communication Permission
I authorise that my child’s school has permission to release all personal information about my child to Sherpa Kids.
Sherpa Kids East Cork Payments
I understand that I will be billed weekly in arrears and have 7 days to pay my bills
Ongoing non payment of bills will result in possible exclusion of my child(ren) from the service until all bills have been paid.
Changes must be made by 1pm on Friday for the next week to firstname.lastname@example.org or I will be charged for all bookings existing,regardless of attendances.
Sherpa Kids Childcare Declaration
Ihavereadandunderstoodthepoliciesreferredtoabove and in the Parent Handbook. I have read the terms and conditions that apply to payments when booking with Sherpa Kids.
I acknowledge that two weeks’ notice, in writing, must be provided if a child is to be withdrawn from care or there is a significant change required to the days of care. Otherwise a two-week fee is payable based on the previous booking.
I acknowledge all information I have provided on this form is true and correct. I am aware it is my responsibility to advise Sherpa Kids immediately of any change in the above information.
NCS RULES - You will be awarded a maximum number of hours on the scheme, for Junior and Senior infant children it is 17 hours a week and from 1st class up it is 12 hours a week.
- You need to decide how many of those hours you will avail of. You must then contact Deirdre with your child's CHICK number and number of hours you require. A contract is then set up on the NCS Hive and must be confirmed by yourself.
- Once this contract is confirmed, your weekly bill, regardless of attendance, will be set to this amount. So, for example, if you need 10 hours a week at €6.50 an hour, and your discount with the NCS is €3 an hour, then you will pay €3.50 an hour or €35 per week. If your child is absent or under attending, you will still pay €35.
- Due to the administration and man hours involved in the NCS , once you commit to a certain number of hours under this scheme you will no longer be billed based on attendance.
